The medallion has 3 charges. While wearing it, you can use an action and expend 1 charge to cast the Detect Thoughts spell (save DC 13) from it. The medallion regains 1d3 expended charges daily at dawn.

Detect Thoughts gives you a tool to probe into the minds of those around you in a way no other feature in 5th edition does. Having a Medallion for free uses of it fits the mentalist or psychic character archetype perfectly.

The item technically casts the spell, not you, for the sake of class abilities that care about casting Divination spells, such as Diviner’s Expert Divination.
Many tables won’t have a physical d3 (which does exist). D6s can be used in place, with two faces on the d6 representing one face of the d3.
